Fill the Questionnaires Form for a Procedure
This guide provides step-by-step instructions on accessing, completing, and submitting the questionnaire form for a procedure.
Understanding Terminologies
Questionnaire Form
A questionnaire form is a structured document to collect relevant patient information before a procedure. It includes questions about medical history, lifestyle, and specific health conditions to ensure safe and personalized treatment.
Fill the Questionnaire Form Through the Patient Profile
- The patient can fill the questionnaire form through the Patient Profile accessible via three points.
- Click anywhere on the patient's data in the patient listing, or click the three dots under the action column next to the specific patient and select '’View Patient Profile.’’
- On the Patient Profile, patients can access a comprehensive summary of all their details. To view questionnaire forms to fill, simply click on the ‘’Paperwork’’ tab.
- The Paperwork section in the Patient Profile displays all bookings and pending paperwork. Click ‘’Fill Questionnaire’’ to complete the required forms for your booked service.
Note: Only booked services with attached questionnaires appear in paperwork, provided the procedures have not yet been made for these bookings.
- Fill out the questionnaire form, then click ‘’Finish’’ to save your responses.
The questionnaire form has been successfully filled through the patient profile!
Fill the Questionnaire Form Through the Email
- When a booking is made for a service that includes an attached questionnaire, the patient will receive an email with the booking details and any required paperwork. To complete the questionnaire, click "Fill Questionnaire" in the email.
- The patient will be directed to the Bookings tab in their profile to complete the questionnaire. After filling out the form, click "Finish" to save it.
The questionnaire form has been successfully filled through the email!
Fill the Questionnaire Form While Creating the Health Procedure
- The user can fill the questionnaire form while creating a health procedure accessible via five entry points.
- From the EMR Board Tabs, click on the “MD Room” tab and select "New Health Procedure.’’
- From the EMR Board Tabs, click on the “Provider Room” tab and select "New Health Procedure.’’
- From the EMR Board select a booking that is in the ‘’Check-in’’ step then press the ‘’Next’’ button to move that booking to the ‘’In Procedure’’ phase.
- From the EMR Board Tabs, click on “Patients.”
- From the EMR Board Tabs, click on “Bookings.’’
- From the list of all bookings, click on three dots under the action column and select "New Health Procedure" for the specific booking.
Note: Clicking the "New Health Procedure" button from any entry point will take the user to step 1 of creating a health procedure. If patient information and booking details are already entered, the user will go directly to Step 3 of creating a health procedure.

- From the EMR Board Tabs, click on the “MD Room” tab and select "New Health Procedure.’’
- Complete 4 steps of creating a health procedure. In step 5, select the “Questionnaire” relevant to the procedure from the dropdown menu. You can choose as many questionnaires as needed. After selecting a questionnaire, click “Next.”

- After the questionnaires are selected, the user can have the patient fill out the questionnaires by clicking on “Fill Questionnaires” if the patient has already checked in at the Med SPA facility to get the procedure done. Alternatively, the patient can log in to their patient portal and fill out the questionnaires before the procedure is performed.
- After the questionnaires are filled, click “Finish” to proceed.
Note: It is mandatory to fill out the questionnaires before the procedure begins.The questionnaire form has been successfully filled while creating a health procedure.
Fill the Questionnaire Form While Creating the Cosmetic Procedure
- The user can fill the questionnaire form while creating a cosmetic procedure accessible via five entry points.
- From the EMR Board Tabs, click on the “MD Room” tab and select "New Cosmetic Procedure."
- From the EMR Board tabs, click on the “Provider Room” tab and select "New Cosmetic Procedure.’’
- From the EMR Board select an appointment or booking that is in the ‘’Check-in’’ step then press the ‘’Next’’ button to move that booking to the ‘’In Procedure’’ phase.
- From the EMR Board Tabs, click on “Patients.’’
- From the EMR Board Tabs, click on “Bookings.”
- From the list of all bookings, click on three dots under the action column and select "New Cosmetic Procedure" for the specific booking.
