Consent Forms
A Consent Form is a legal document that patients sign to authorize specific esthetic procedures. It outlines the procedure details, including potential risks and benefits, ensuring patients make informed decisions about their care. By signing consent forms, patients acknowledge their understanding of the procedure, providing legal protection for both the patient and the Med SPA while promoting transparency and trust.

Create a Consent Form
- The user can create a consent form by navigating to the Clinical Documentation App from two entry points.
- Once you're in the 'Clinical Documentation App,' the 'Questionnaire' list will open by default, as it is the first option in the dropdown menu for Clinical Documentation. From the 'Clinical Documentation' dropdown, select 'Consents.’
- To create a consent form, click the ‘’New Consent’’ button in the top right corner of the screen.
- Fill in the required information, including Name and Body of the consent. After adding the details, click 'Create Consent' to save it.
The Consent Form has been successfully created!
Edit a Consent Form
- From the list of added consent forms, click the three dots in the action column and select 'Edit' for the specific consent form you want to edit.
- Edit the information you would like to change and click "Update Consent."
The Consent Form has been successfully edited! Note: Consents can also be edited by clicking the "Edit Consent" button in the top right corner of the View Consent Page.
Attach a Consent Form to a Service
- In Step 2 of creating a new service, a dropdown to select a consent form will appear. Choose the relevant consents to attach to the service you are creating. You can attach one or multiple consent forms to the service.
- After selecting the relevant consent forms for the service, click “Next”. At Step 3, click “Submit” to save the updates.
The Consents has been successfully attached to the service!
Attach a Consent Form to a Procedure
- In Step 6 of creating a procedure, whether health or cosmetic, attach relevant consents to the procedure from the dropdown menu. You can select as many as needed. After selecting a consent form, click “Next.”
- After adding the relevant consent forms, the user can have the patient sign the consent form by clicking on “Sign Consents.”
- Then press the "Click to Sign" button, if the patient has already checked in.
- A blank screen will appear for the patient to provide their electronic signature. Once the signature is completed, click "Save". Alternatively, the patient can log in to their patient portal and sign the consent form independently.
- The patient's electronic signature will appear below the consent form on the screen. Once confirmed, click "Finish.’’
